Post Concussion Symptom Severity Scale (PCSS) This is the Post Concussion Symptom Severity Scale. If you have had an injury compatible with concussion (car accident, neck whiplash, fall, trauma to head and face, sports trauma, etc) and present with at least one of the 22 symptoms after the accident, you need to consider that you may have had a concussion. You should grade your symptoms from 0 to 6, where 0 is ‘no symptom’, 1 and 2 ‘low intensity symptom’, 3 and 4 ‘moderate intensity symptom’, and 5 and 6 ‘severe intensity symptom’. What we understand about it has been changing in the last few years and the approach for assessment and treatment is constantly being updated.
One of the most common misunderstandings regarding concussion is its cause. We used to think until recently, that it was caused by a direct trauma to the head, which caused the brain to ‘bang’ against the skull causing a sort of bruise on it. We know now this is not true. What really happens is a fast stretch in the brain cells secondary to a movement of acceleration and deceleration of the brain tissue, with or without direct head trauma. This means that you can have a concussion, even without hitting your head. This can occur during the movement of a neck whiplash, for instance. A HEADACHE CAN TAKE OVER YOUR LIFE. We have had patients with a constant severe headache for two years, have it resolved fully to a pain free status, with one gentle treatment. There are many ways skilled hands-on treatment can help with many types of headaches and migraines. First we take a medical history, and then perform the appropriate neurological, physical and osteopathic examination. Each assessment and treatment is carefully optimised for your needs. You can have a ‘click’ or choose to have more gentle treatment approaches. It is your choice. Whiplash is defined as an “acceleration-deceleration mechanism of energy transfer to the neck”. It could be divided into two parts. First, the impact on the neck creates hyperextension (excessive motion that creates a locking of the facet joints and stretching of the anterior/front of the neck soft tissues), then, there is a too-fast reversal motion called hyperflexion. This is an unusual case study as the child who fell is my daughter. I was able to assess and treat her immediately. If your child falls and hits their head it is important to go immediately to your GP or Accident and Emergency for concussion/TBI assessment. Then once their condition is stable you may like to look at complementary treatment options.
A Brief Case Study: Seven year old girl fell 3.5 metres vertically onto her head in the April school holidays Said she felt ‘sleepy’ at first (then yelled and cried loudly) Osteopathic assessment took place less than five minutes after the injury. The whole top of her head felt very slightly depressed and her cervical spine was side-bent to the left and locked. Neurologically all tests appeared clear. Gentle treatment of the neck and head to assist normal range of motion and function of the membranous tissue of her cranium resulted in her feeling ‘fine’. She was running around happy so there was no indication that she needed a CT scan. However, two hours later during a walk up a hill she had a headache, so we suspected concussion and she went in the mild Hyperbaric Oxygen chamber. This works by reducing the oedema in the brain tissues and also supplies 5-7 times the usual amount of oxygen to the mitochondria of the neurones which enables more of them to start firing again. This quickly cleared the headache during the session. She also had some symptoms involving feeling unusual anger and fatigue, also indicating a concussion Daily osteopathic and mild hyperbaric oxygen sessions for the next three days kept her feeling symptom free and six days after her fall she won her school’s year 3 and 4 cross country trial, even against the boys!
